Overview

This article addresses the most common Certificate of Insurance (COI) issues, including upload errors, verification delays, and Additional Insured requirements. Most onboarding delays are caused by COI formatting or content issues.


Why is my COI being rejected or showing an error?

COIs are typically rejected for one or more of the following reasons:

  • Required coverage types are missing
  • Coverage limits do not meet network requirements
  • The network entity is not listed as an Additional Insured
  • The document is expired, incomplete, or unreadable

Additional Insured vs Certificate Holder (Important)

To meet requirements:

  • The network entity must be listed as an Additional Insured
  • Listing the network only as a Certificate Holder is not sufficient

Your COI must either:

  • Have the “Additional Insured” box checked, or
  • Explicitly list the network entity as an Additional Insured in the description section

What happens after I upload my COI?

After upload:

  1. Your COI enters Pending Validation
  1. Pruuvn® initiates verification
  1. Your insurance agent may be required to confirm the certificate

Until validation is complete, your COI may remain pending.


Does my insurance agent need to send the COI?

In many cases, yes.

For verification, COIs are often required to be:

  • Sent directly from the insurance agent
  • Delivered to the designated COI verification contact

Uploading a COI without agent confirmation may delay approval.


Why does my COI stay in “Pending” status?

Common reasons include:

  • Agent has not responded
  • Additional Insured language is missing
  • Coverage limits need clarification
  • Required endorsements are missing

Following up with your agent is often the fastest resolution.


Can COI issues affect my account or access?

Yes. If your COI does not meet requirements:

  • Network eligibility may be restricted
  • Your profile may be temporarily limited
  • In some cases, login access may be affected

Access is typically restored once compliance is resolved.
